Self-Management Education Program
Our Diabetes Self-Management Education program is accredited by the American Diabetes Association and includes a variety of instruction.

Evaluating and Assessing Your Individual Needs

Your initial visit with our dietitian will consist of evaluating and assessing your individual needs and will include
:    

      
  • Carbohydrate Counting
  • Cholesterol & Blood Pressure Improvement through Diet
  • Fiber
  • Good Fats vs. Bad Fats                                                                                                                
  • Getting to a Healthy Weight                                   
  • Reading Food Labels…and more!

 Your initial visit with our Nurse will consist of evaluating and assessing your individual needs and will include:

  • Insulin & Medication
  • Checking Blood Sugars        
  • Complication/Risks Reduction
  • Sick Day Guidelines
  • Signs and Treatment of Low Blood Sugar
  • Exercise…and more!

 Educational Group Classes
The Diabetes Self-Management Education (DSME) classes are held in the friendly atmosphere of our on-site classroom, where patient participation is encouraged and is taught by our Diabetes Nurse and Dietitian.  The classes cover such topics as:

     
  • Diabetes: What is it?
     
  • Nutrition & Exercise
  • Medications & Monitoring                                       
  • Complications of Diabetes                                      
  • Managing High & Low Blood Sugars
  • Understanding Your Lab Work

 This program requires a physician referral. Upon receiving the referral from your physician, our department will contact you to schedule your individual appointments with our Diabetes Registered Nurse and Registered Dietitian.

Preparing for Your Individual Visit

Forms you may need to complete are listed below.  Please call the Diabetes Program at 336-401-8419 for specific forms that you will need to download and complete for your visit.  Forms can also be mailed to you. 

Also, Please Bring the Following Information with You

  • All Medicare, Medicaid, and/or Other Insurance Cards
  • Blood Sugar Testing Supplies and/or Insulin and Syringes
  • Completed History and Nutritional Forms (see above)
  • List of All Medications
  • Proof of Household Income (OPTIONAL – please see below)

Payment Options? YES... 

  • As a courtesy to you, we will file claims for Diabetes Self-Management Education with Medicare, Medicaid or any other insurance
  • Since no insurance guarantees payment, providing proof of household income will allow us to slide fees on an income-based scale when applicable. This income-based fee option may reduce the costs for services not paid by your insurance
  • If needed, we will assist you in contacting your insurance provider to determine coverage for our services
