How can I obtain a Death Certificate?
To obtain a Death Certificate by mail, submit your request to the address listed in the General Information section for Birth and Death Records. Requests for Death Certificates must include the full name of the deceased and the date of death. To obtain a Death Certificate in person you must present a valid form of identification.  To obtain a certified copy of a death certificate, simply click the below link and follow the instructions.
