Pregnancy Care Management
Pregnancy Care Management services are available to pregnancy and postpartum women receiving Medicaid who have risk factors which may put them at risk for poor birth outcomes.
Care Coordination for Children (CC4C)
CC4C is a free and voluntary program that helps families like yours find and use community services.
Postpartum Newborn Assessments
Postpartum Newborn Assessments are conducted by a Registered Nurse who will visit Medicaid-eligible mothers and their newborn infants within 14 days of discharge from the hospital.
SIDS/Grief Counseling provides a counseling and support network to Surry County families who suffer the loss of a child due to Sudden Infant Death Syndrome (SIDS).
